Service description:

Headway Portsmouth and South East Hampshire is a registered charity providing information, advice and support to adults with acquired or traumatic brain injury, their family, and carers to enable the individual with brain injury to return to community living.

Our non residential Cognitive Reablement Programme is designed to support community rehabilitation following brain injury.  This programme focuses on the development of individual goals using established CRT techniques.

Modules include:

Living with a brain injury; Behavioural modification techniques; Strengthening relationships; Thinking about our health and positive well being; Improving our communication; Better planning and organising; Strategies for improving our memory; Managing money with confidence

Exclusion criteria:

Under 18 years of age. Patients requiring nursing care or manual handling unless accompanied by a Personal Assistant. Psychotic or acute mental illness 

Catchment areas

Portsmouth and South East Hampshire including the boroughs of Petersfield, Bedhampton, Havant, Gosport, Fareham, Wickham and Hayling Island.

Referral process:

Referrals can be made through the person’s GP, Consultant, other health care professional, insurance company, case management company, social worker, Adult Services.

Headway operates a charging policy in line with Adult Services Contributions Policy and referrals may be required to undertake a financial assessment.

Self referrals are also accepted.


Headway – Community Social Outreach Services

Main Speciality:  Outreach Services in the community following traumatic or acquired brain injury linked to above service. All contacts, exclusions, catchment area, referral process etc. are similar.

Service:

Our Community Social Outreach Services promotes peer support, personal development, social activities and reduces social isolation and exclusion following brain injury.  Activities include: 

Group discussions facilitated by Headway staff; Peer lead activities; Board and card games; Quizzes; Sports activities including tennis and Boccia; Organised events. 

Venues include Gosport, Portsmouth and Petersfield 


Headway – family members support group

High level service description:

The family members support group provides opportunities for family members and carers who may be in similar circumstances to meet and share experiences with others knowing that you are not alone. The service includes: group discussions; day trips; pamper days; organised events; Brain Injury Awareness Training.

Exclusion criteria: Under 18 years of age.
