Service description:
The Community Neurological Services consist of:
- Community Neuro Rehabilitation Team (OT, PT, SLT, Psychology and Dietician)
- Neuro Early Supported Discharge Team
- Neurological and Long Covid Vocational Rehabilitation Team
- Neurological Physiotherapy Rehab Gym
- Multidisciplinary Spasticity Clinic
- Transition Therapy Team
- Specialist Nursing services (MS, Parkinson’s, Epilepsy)
- Adults Speech and Language Therapy
- Community Neuropsychology
Exclusion criteria:
Non neurological Conditions (i.e. trauma/burns without ABI)

Catchment areas:
These are dependent on individual service criteria however patches covered could consist of: Southampton City, West Hampshire and Portsmouth City. Out of area referrals (NCR’s) are accepted on a case by case basis.
Referral Process:
GP’s, Consultants, therapists and nurses. Self-referrals are accepted for some of our services.
